The Intake coordinator position will facilitate the referral intake process for potential patients/clients, including receiving referrals, verifying benefits, gathering necessary medical information, and coordinating care with the patient/client, their family, and the referral source for a smooth transition into agency care. In addition this position provides clerical and communication support, assists with personnel file maintenance, and promotes the Boost mission by ensuring effective communication and maintaining confidentiality.

What you’ll be doing:
●       Answers incoming calls from potential patients/clients, healthcare providers, and other referral sources to assess eligibility for agency              services.  
●        Receives and appropriately documents new referrals in a timely manner and completes the benefit verification process. 
●        Once the Clinical Manager accepts a new referral this position will be in charge of gathering medical information/records and other                   pertinent paperwork from referral sources to maintain comprehensive intake files. 
●        Participates in coordinating care with Clinical Manager, patients/clients, and the Interdisciplinary Group/Team to ensure seamless                     patient/client care transition. 
●        Provides effective communication to patients/clients, staff members, other health care professionals, and referral sources to foster                   and maintain positive relationships. 
 ●       Provide clerical support which may include preparation of admission, orientation, and new hire packets; monitoring and ordering office             supplies and forms, typing, special projects, filing and copying.
●        Facilitate the communications system which may include: mail distribution, memos, answering incoming calls, and document and                    disseminate appropriate messages.
●        Assist with maintenance of personnel files, obtaining signed doctor orders, scheduling clinicians for client visits
●        Promote Boost’s philosophy and mission by presenting a positive image to patients, families, physicians, and community agencies.
●        Provide computer support and data entry. 
●        Provide effective communication to patient/family, team members, and other healthcare professionals and maintain confidentiality.
 
 
What we’re looking for:
●        High school graduate
●        One (1) year general office experience in Home Healthcare 
●        Excellent computer skills required
●        Demonstrated interpersonal and organizational skills
●        Reliable transportation and valid auto liability insurance
●        A passion to serve and help others live their best lives possible.
●        Active CNA license beneficial
Salary Range:
●       41,500 to 52,000 per year
Compensation: $20.00 - $25.00 per hour
